"The Childbirth and Childcare Lump-Sum Grant" is paid when an insured person gives birth and "the Dependents' Childbirth and Childcare Lump-Sum Grant" is paid when a dependent family member gives birth. Don't forget to add the newborn as a dependent in these cases.

Request a Childbirth and Childcare Lump-Sum Grant

When using the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions

Submit the documents on agreement to use the system to the medical care institution or other facility where you plan to give birth. (You need not take any action with respect to the Health Insurance Society. For details, check with the medical care institution or other facility where you plan to give birth.)
Even if you use this system, you must apply to the Health Insurance Society to receive additional benefits.

    (Documents demonstrating that the details are the same as those on the special invoice submitted by the medical care institution or other facility to the Health Insurance Society and, if giving birth at an institution that is a member of the maternity medical care compensation scheme, showing printed or stamped text certifying that the childbirth took place at a member institution)

Notes: Even if childbirth expenses are less than the amount of the Childbirth and Childcare Lump-Sum Grant to be paid, the amount paid will not change.

When using the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf

If you would like to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf, submit the following application to the Health Insurance Society in advance.

Deadline: In advance
Applies to: Insured persons and dependents expecting childbirth up to two months in the future who wish to use the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf
Submit to: Pfizer Health Insurance Society

If you paid the entire amount of childbirth expenses to the medical care institution

Submit the following application to the Health Insurance Society if you have not used the system of direct payment of Childbirth and Childcare Lump-sum Grant to medical institutions or the system of receipt of the Childbirth and Childcare Lump-sum Grant directly by the medical institutions on your behalf or you have given birth overseas.

Notes:

The following documents must be attached for a childbirth overseas:

  • Documents issued by the overseas medical care institution or other facility where the birth took place (birth certificate, receipts, etc.)
  • Japanese translations of the above
  • A copy of document(s) attesting to the fact of travel overseas (passport, etc.)
  • A letter of consent permitting inquiries with the overseas medical care institution or other facility where the birth took place regarding the fact of the birth, details, and so forth

Receiving a loan for childbirth expenses

To ensure that the insured person or dependent can deliver their child without worrying about the financial burden, the Health Insurance Society offers a loan equivalent to 80% of the amount of the Childbirth and Childcare Lump-Sum Grant.
Based on your application, the loan will be deposited to your designated account one month before the expected date of childbirth. Please note that the loan amount will be deducted from the Childbirth and Childcare Lump-Sum Grant.

  • Document stating the expected date of childbirth of the person eligible for the loan (maternal and child health handbook, etc.)
  • If the person eligible for the loan is four or more months pregnant and needs to make a temporary payment to a medical care institution, document proving that she is four or more months pregnant (maternal and child health handbook, etc.) and a claim or receipt from the medical care institution with a breakdown of the expenses required for childbirth

Notes: The loan cannot be combined with the use of the system of direct payment of Childbirth and Childcare Lump-Sum Grant to medical institutions. Please note that users of the system will be asked to repay the loan.

Caution

If you are a Pfizer employee, please apply by performing the procedure below. After you apply, the necessary documents will be sent to you. Please refer to the FAQ regarding the post-application process.

  • (1) Use Workday to register family member information.
  • (2) Electronically apply (Navit) for dependency.
  • (3) After you apply, the Pfizer business subcontractor will mail the necessary documents to your home.
    (The documents will be mailed two to three days after the application is complete.)

  • If the spouse has income, a copy of the spouse’s certificate of income and withholding tax for the previous year

Applies to: Insured persons who have additional family members to enroll due to childbirth

Notes: If both parents work, as a general rule the child will become a dependent of the person with the higher income. If you want the child to be a dependent of your spouse, you do not need to report to the Pfizer Health Insurance Society.
