Do the following each time you move or transfer between jobs:
Enter your new address on the reverse side of your health insurance card after crossing out your previous address with double lines.

Insured Persons
If you submit a change of name to your employer, the change will automatically be updated at the Health Insurance Society. You don't need to submit documents to the Health Insurance Society.
Dependents or Voluntarily and Continuously Insured Persons
Please inform the Health Insurance Society of any change of address, so we can keep you informed of necessary information.
