I had glasses created for amblyopia (lazy eye) and then applied with the Health Insurance Society. When I tried to apply with the local government for the difference in the amount, I was told that I need the original instructions for creating the glasses and receipt. Can you return these to me?

If you have not yet applied, when you submit the Application Form for Medical Care Expenses, please attach a sticky note that indicates "why you need the items returned" and "the names of the documents that must be returned."

If you paid the entire medical care cost up front

	I am a Pfizer employee, and—to add a family member as a dependent—I used Workday to register the family member information and also applied electronically (Navit). Please tell me about the post-application flow as well as how long it will take for the insurance card to arrive.
