You may apply for medical care expenses if your child is less than nine years old and the glasses are used for treatment purposes. Please attach the documents below to apply.

Maximum amount if the cost-sharing percentage is 30%: 36,700 yen × 1.048 × 0.7 = 26,922 yen
Maximum amount if the cost-sharing percentage is 20%: 36,700 yen × 1.048 × 0.8 = 30,768 yen

If you paid the entire medical care cost up front

	I am a Pfizer employee, and—to add a family member as a dependent—I used Workday to register the family member information and also applied electronically (Navit). Please tell me about the post-application flow as well as how long it will take for the insurance card to arrive.
