I plan to leave my job this month and am considering enrolling in voluntary and continuous insurance. Can I use the Certificate of Application of Maximum Copayment Amount for Health Insurance that has already been issued to me, or is it necessary to reapply?

It is not necessary to reapply for a Certificate of Application of Maximum Copayment Amount. If you have already had this certificate issued and you submit an Application Form for Certification as Voluntarily and Continuously Insured Person, a new Certificate of Application of Maximum Copayment Amount will be issued.
Note that, if you become a voluntarily and continuously insured person, the code and number indicated on your current Certificate of Application of Maximum Copayment Amount will change, so you will no longer be able to use your current certificate.
