I lost my Certificate of Application of Maximum Copayment Amount after using it. What should I do?

If you have lost or cannot otherwise return the certificate, it is necessary to submit a "Notification of Certificate of Application of Maximum Copayment Amount Loss." Please download the application form from "Application forms," fill it out, and then apply by either sending the form as an email attachment or by mailing it to the Health Insurance Society.
