I have been re-employed. Are there any necessary procedures?

Upon withdrawing, please send the items below to the Pfizer Health Insurance Society.
(1) Notification of Disqualification as Voluntarily and Continuously Insured Person（This can be downloaded from "Application forms."）
(2) The insurance cards of voluntarily and continuously insured persons (including family members' cards)
(3) Copies of insurance cards issued by the place of re-employment or a copy of the certification of eligibility
* In addition to insurance cards, also return the "Certificate of Application of Maximum Copayment Amount" as well as any other certificates issued by the Health Insurance Society.
* If there is an insurance premium refund, the Health Insurance Society will perform the repayment procedure after the above items have been received.

When you leave your employer

	I am a Pfizer employee, and—to add a family member as a dependent—I used Workday to register the family member information and also applied electronically (Navit). Please tell me about the post-application flow as well as how long it will take for the insurance card to arrive.
	I will soon be hospitalized. Are any procedures necessary?
	The Childbirth and Childcare Lump-Sum Grant (420,000 yen) and additional amount (50,000 yen) were transferred to my account as a salary payment. Is it possible to have a certificate that shows the details issued?
