Is it okay to submit an "Application Form for Certification as Voluntarily and Continuously Insured Person" before leaving my job? Also, can I receive my insurance card in advance?

Please submit the application form either before leaving your job or within 20 days after leaving it.
Your new insurance card will be sent on or after the qualification date (the day after you leave your job), so it will reach you two or three days after you leave your job. (If you applied after leaving your job, you will receive your card two or three days after your application form arrives.)

When you leave your employer

	I am a Pfizer employee, and—to add a family member as a dependent—I used Workday to register the family member information and also applied electronically (Navit). Please tell me about the post-application flow as well as how long it will take for the insurance card to arrive.
